SCREENING DESCRIPTION

Health Assessments are performed by a certified health coach.  Participants will fill out a health assessment form.  The process takes about 15-20 minutes.  No health coaching is provided.  The assessment is for informational purposes only.  Participant information is kept confidential per HIPAA guidelines.  Following the assessment, participants have the options to schedule an appointment should they wish to receive health coaching service so that they can achieve their health goals.

 

Framework of the health assessment will include:

1.     Collection of specific paradise (dreams, hopes & visions) and the specific result that participant wants in the area of their life so that a health plan/goals can be created

2.     To help clarify the current reality of where participant is so that the health coach can have an idea of what we have to work with

3.     Identifying all the obstacles that are slowing participant down or stopping them.  We can turn those obstacles into projects and participant achievement of that paradise

 

Overall objective of the health assessment is to 

-       Help participants to know that through health coaching support, motivation and accountability they can identify health goals and achieve them. The results will be lowering their risk of disease or put their illness in remission, which in-turn increase productivity, reduce absenteeism as well as improve health outcomes and overall life quality.
